We have interviewed Emergency Room Doctors about what information they want right away. All of the needed health information is included in the Medical History form. Make sure that your minor children are also included in their own files.
Once you have filled out and completed the History, Print it out and put the proper signatures where indicated. Use Black ink so when you scan the finished signed document, it will be visible and easy to read.
All documents must be scanned and then uploaded to the proper file. The program accepts all the common picture formats, IE jpg, png, gif etc. If you don't have a scanner you can use a digital camera, or a cell phone that has camera capabilities. You can also take your forms to stores that scan your documents and loads them on a CD or a DVD.
Don't forget to use the "Save As" function to give the finished Medical History a different name to prevent overwriting the blank file.
